If you have lost or damaged your CBT4 (pink slip) issued after completing a CBT or Direct Access, please use the form below to request a reprint.
Please note that we DO NOT post these out. You will be contacted to arrange collection. Collection will usually take place over the weekend when CBT is taking place!
Please note that if your last CBT/DA was over two years ago, we cannot replace it as they are only valid for two years.
